we can begin providing therapy for your child, you
will need to complete a few forms in order to
initiate the referral process. For your convenience,
we have provided these forms on our website in PDF
for you to download and print on your computer. Or
you can call our office at 941-379-3725 and we will
be happy to mail you a Welcome packet. Please mail
or fax (941-377-1131) your completed forms to us. As
soon as these forms are received at our office, a
therapist will call you to discuss your application
and set-up an appointment. If you have questions,

We are providers for Medicaid,
the Early Intervention Program (EIP), and many
insurance companies. If you would like assistance
with insurance billing, our office will assist you
by verifying your insurance coverage. Please call us
to determine if we are in your network. We also
strongly suggest that you check with your insurance
company to determine possible benefits or
exclusions. A doctor’s prescription for therapy is
required when seeking insurance coverage.
